Northeast Health Wangaratta (NHW) is a leading healthcare provider in the Central Hume region, offering a wide range of acute, sub-acute, aged care, and community services. We are currently seeking an experienced Nurse Practitioner to work within our Residential In Reach Service, supporting aged care residents across 17 facilities within the region. The Residential In Reach Service provides hospital-level assessment and short-term management of acute medical conditions, reducing the need for emergency department visits and hospital admissions.
