This full-time position is based at our Care Home in Klemzig, SA, with travel required to our other Care Homes in Kilburn, Gilles Plains, Christies Beach, and Hahndorf. The role involves covering planned and unplanned leave for the Clinical Care Coordinator and Nurse Mentor positions.

Key Responsibilities

Providing Clinical Leadership

  • Deliver clinical expertise and leadership to care teams across multiple facilities.
  • Assist the Clinical Care Manager in overseeing clinical indicators to achieve optimal outcomes.

Resident Assessments and Care Planning

  • Manage all assessment, documentation, and submission of AN-ACC requirements.
  • Oversee care planning and review processes to ensure compliance with best practices.
  • Support new resident admissions by conducting care assessments and consultations with families.

Compliance and Risk Management

  • Ensure accurate documentation aligned with organisational and legislative requirements (AN-ACC, resident records, staff time tracking).
  • Manage clinical risk and promote continuous improvement across facilities.

Team Support and Mentoring

  • Support the clinical team in adhering to policies, procedures, and IT system requirements.
  • Influence and develop clinical leadership skills among staff members.
  • Provide input on facility team rosters to maintain an optimal skill mix.

What You Bring

  • Willingness to work across all Infinite Care Homes in SA: Klemzig (base), Kilburn, Gilles Plains, Christies Beach, and Hahndorf.
  • Minimum 1 year experience in a Care Coordinator or leadership role.
  • Availability to work Monday to Friday – day shifts.
  • AHPRA Registered Nurse Div 1 (unrestricted registration).
  • Postgraduate qualifications in aged care (advantageous but not essential).
  • Extensive knowledge of Aged Care clinical assessments, AN-ACC, and accreditation compliance.
  • Strong IT and documentation skills.
  • Ability to manage multiple stakeholders and high-pressure situations.
  • Valid NDIS Workers Screening Clearance (or willingness to obtain).
